Everton boss David Moyes criticised his defence for conceding a shabby goal in Sunday's 1-1 draw with Reading at Goodison Park.
Joleon Lescott's own goal gifted the Royals a first-half lead before Andrew Johnson rescued a point for the Toffees nine minutes from time.
Moyes insisted the opening goal was offside, but was unhappy with the defending which allowed Steve Hunt to ghost in unmarked from a free-kick.
He said: "We made some poor decisions in that first half and then we conceded a terrible goal.
"I felt it was a shabby goal that we conceded. It was offside, and the linesman was right there and didn't give it, which is disappointing.
"But there was a lot wrong with the way we defended that free-kick. We had worked in the week on filling our goal-line with players and we were deliberately deep.
"But it was not set up the way we planned and, although I take responsibility for the system, it was not how it should have been put into operation.
"But their lad was offside when the ball was kicked, ran onside to get his header in and it should have been penalised."
